How to Set Up Lab Ordering for a Telehealth Hormone Practice

Lab ordering is the operational backbone of a hormone optimization practice. Unlike general primary care, where labs are ordered sporadically, hormone optimization requires comprehensive baseline labs before starting any therapy, repeat monitoring labs at 6–8 week intervals, and ongoing quarterly or semi-annual panels for maintenance patients. Building your lab infrastructure correctly from the start determines whether your clinical workflow is smooth or chaotic.

For telehealth practices, the critical requirement is direct-to-patient lab access — meaning patients can get their labs drawn at a location near them without needing to visit your clinic. The two major national networks are LabCorp and Quest Diagnostics, both of which have patient service centers in virtually every metropolitan area and many rural locations across the US. As a licensed prescriber, you can order labs through both networks and have results sent directly to you in your EHR.

For patients who prefer home-based collection, direct-to-patient lab services like Labcorp OnDemand, Everly Health (which processes through CLIA-certified labs), and Ulta Lab Tests offer at-home collection kits for most hormone panels. These are particularly valuable for patients in rural areas or those who have difficulty getting to a patient service center.

The specific hormone panel you order at baseline should be comprehensive: total and free testosterone, estradiol, progesterone (for women, timed to cycle), DHEA-S, SHBG, LH, FSH, free T3, free T4, TSH, reverse T3, thyroid antibodies (anti-TPO, anti-TG), fasting insulin, fasting glucose, HbA1c, complete metabolic panel, CBC with differential, lipid panel, vitamin D (25-OH), ferritin, hs-CRP, and PSA for men over 40. This panel gives you the full hormonal and metabolic picture before any intervention.

Establish your lab accounts before seeing your first patient. Both LabCorp and Quest require provider credentialing — allow 2–4 weeks for this process. Your EHR should have direct integration with at least one major lab network to allow electronic ordering and result delivery.
