One of the most common questions practitioners ask when evaluating a telehealth practice build is how their specific credential impacts their ability to practice independently, prescribe broadly, and operate without oversight. The answer varies significantly by credential and state — here is a direct comparison to help you understand your starting position and your strategic options.
MDs and DOs have the broadest prescribing authority in all 50 states and the most straightforward path to independent telehealth practice. A physician can prescribe Schedule II–V controlled substances in any state where they hold licensure, practice independently without a supervisory agreement, and directly credential with labs and pharmacies as the prescribing authority. The primary limitation for physicians building a telehealth practice is often business infrastructure — the business and operational decisions that medical school did not address.
Nurse Practitioners in Full Practice Authority (FPA) states have essentially equivalent independent practice and prescribing rights to physicians for the scope of practice covered by their specialty certification. An FPA-state NP can see patients, diagnose, prescribe Schedule III–V controlled substances, and operate entirely independently. For hormone optimization specifically, the FNP or AGNP credential covers the patient populations most relevant to this practice model. In Reduced and Restricted Practice states, a collaborative physician agreement is required — which is a manageable overhead for most NPs, but adds a layer of administrative complexity.
Physician Assistants currently require a supervising or collaborating physician in most states, though the level of oversight required is decreasing as states modernize their PA practice acts. The Optimal Team Practice model — which removes the requirement for a formal supervisory agreement and allows collaborative practice — has been adopted by a growing number of states.
