For practitioners in functional medicine and hormone optimization specifically, the employed model is particularly restrictive: the comprehensive, personalized, time-intensive approach required for excellent hormonal care is almost entirely incompatible with the 15-minute appointment model of insurance-based employed practice.
The transition to independent practice typically happens in phases. Phase 1 is building the infrastructure while still employed — establishing the legal entity, obtaining additional state licensures if needed, setting up the EHR and operational systems, and beginning the patient acquisition groundwork (Google Business Profile, content creation, referral network outreach). This phase is done outside of working hours and does not yet generate patient revenue. Phase 2 is the soft launch — typically starting to see patients on evenings and weekends while maintaining employed position, generating early revenue, testing systems, and building clinical confidence in the independent workflow. Phase 3 is full transition — leaving employment once the independent practice generates sufficient revenue (usually 3–6 months of the soft launch phase for practitioners who build systematically).
