Not all compounding pharmacies are equal, and the differences matter clinically and legally. PCAB (Pharmacy Compounding Accreditation Board) accreditation is the highest standard in the compounding pharmacy industry and indicates that the pharmacy has undergone rigorous third-party inspection of its processes, sterility testing, and quality controls. For hormone compounds specifically, you want a pharmacy that is PCAB-accredited, has experience with bioidentical hormone preparations, and operates as a 503A pharmacy (compounding for individual patient prescriptions) rather than a 503B outsourcing facility (which operates under different FDA oversight and primarily serves hospitals and clinics requiring large batches).
The credentialing process with a compounding pharmacy typically requires a copy of your DEA registration, state license(s), malpractice insurance, and NPI number. Most pharmacies complete this process within 1–2 weeks. Many pharmacies also require a signed prescribing agreement that outlines your responsibilities as the prescriber and their quality controls as the compounder.
For your patients, you need to understand the shipping logistics. Most compounding pharmacies ship directly to patients — which is ideal for a telehealth practice where patients never come to a physical office. Shipping timelines, cold-chain requirements for certain compounds, and state-specific shipping restrictions (some states have restrictions on certain compounded hormones) all need to be understood before you prescribe.
