A 60-minute first consultation in a hormone optimization telehealth practice covers an enormous amount of clinical ground — symptoms, history, labs, goals, treatment options, patient education, and next steps. Without a clear structure, consultations run long, key information gets missed, patients leave confused about next steps, and the provider ends the day feeling exhausted rather than productive. A structured consultation template solves all of this.
The first 10 minutes: review the completed intake form and available labs before the patient joins the call. Identify the primary clinical concerns, flag any red flags in the history (cancer history, cardiovascular history, active clotting disorders), and have your initial clinical questions ready. Do not spend the first 10 minutes of the live call reading the intake form the patient already submitted — review it beforehand.
Minutes 10–25 (with patient): Open with an empathetic, non-clinical acknowledgment of what brought them in — “Tell me in your own words what’s been going on and what you’re hoping to address.” Let the patient speak without interruption for 2–3 minutes. This is where you hear the clinical story and establish trust. Then move to specific clinical questioning about the symptoms you want to assess: energy, sleep, libido, body composition, mood, cognitive function, and any specific symptoms relevant to their age and sex.
Minutes 25–45: Review labs (if available), explaining what each marker means in plain language, what you consider optimal vs. just “normal,” and what the clinical picture tells you. If labs are not yet back, explain what you are ordering and why. Discuss treatment options with their rationale, risks, and alternatives — document that this conversation happened.
Minutes 45–55: Discuss the specific treatment plan — what you are recommending, at what dose, from which pharmacy, what the monitoring schedule is, and what the patient should expect to feel and when. Answer questions clearly. Set realistic expectations.
