How to Write a Clinical Note for a Hormone Optimization Telehealth Visit

Clinical documentation in a hormone optimization telehealth practice serves three purposes: it supports the quality and continuity of patient care, it provides the legal record that protects you in the event of a complaint or claim, and it satisfies regulatory and payer requirements for the services you are providing. Building strong documentation habits from your first patient is not bureaucratic overhead — it is foundational practice protection.

A well-structured hormone optimization telehealth note follows the SOAP format (Subjective, Objective, Assessment, Plan) with hormone-specific additions. The Subjective section should capture the patient’s reported symptoms using their own language, any changes since the last visit, current medications and supplements with doses, and how they are tolerating current therapy. The Objective section should document the date and clinical context of the most recent labs, current hormone levels with reference to normal ranges, and any physiological findings relevant to the visit.

The Assessment section is where your clinical reasoning is documented — not just the diagnosis code, but why you are making the clinical decisions you are making.
