How to Start a Hormone Optimization Clinic as a PA: What School Didn’t Teach You

The first legal reality PAs must navigate is prescriptive authority and practice agreements. Unlike NPs who in many states can practice independently, PAs in most states still require a supervising or collaborating physician for prescribing. The specific requirements vary significantly by state — some require formal written agreements with active supervision and chart review requirements, others allow broad independent prescriptive authority with minimal oversight. Understanding exactly what your state requires before you structure your practice is not optional.

Testosterone, while a controlled substance (Schedule III), is routinely prescribed via telehealth across state lines in most states, but the specific regulatory pathway must be mapped for each state you intend to serve.
