Telemedicine Clinic Compliance Checklist: Everything You Need Before Seeing Patients

Compliance is not a bureaucratic obstacle — it is the legal foundation that allows you to practice confidently, protect your patients, and build a sustainable business. Before you see your first telehealth patient, every item on this checklist should be in place and documented.

Legal entity: Your PLLC, LLC, or PC should be formed and registered in your practice state, with an Employer Identification Number (EIN) obtained from the IRS for tax purposes, a separate business banking account opened in the entity name, and any required state business registrations completed.

Licensure: You should hold an active, current license in every state where you intend to see patients. DEA registration should be active and registered to a valid address. State controlled substance registrations (required by some states in addition to DEA) should be obtained. NPI registration should be current and the taxonomy code should reflect your practice specialty.

HIPAA: Security Risk Assessment completed and documented. Privacy Policy and Notice of Privacy Practices written and posted. Business Associate Agreements signed with all vendors handling ePHI (EHR, telehealth platform, labs, pharmacies). Workforce training completed and documented. Breach notification policy and response plan in place.

Clinical systems: EHR configured with your clinical documentation templates, lab ordering integration tested, patient portal active. Telehealth platform tested end-to-end with a practice run. Lab accounts credentialed and active with at least one major network. Compounding pharmacy relationships established with credentialing complete. Patient intake form live and tested. Scheduling system active and bookable.

Insurance: Malpractice coverage active with coverage in all practice states. Business insurance (general liability, cyber liability) active if applicable.
